F502 Conducting an Organization Wide Forms Review

A major Australian Government Department took on the challenge of reviewing all of its public-use forms with a view to making them far easier for the public to fill out. Because of time constraints and a large number of forms, much of the review was conducted by previously untrained staff. In this session we will look at: what the reviewers were taught about improving forms; problems the public had with existing forms and why they were making so many errors; the causes of the problems and what was done to correct them, including a totally new approach to layout; how usability testing and error analysis was used to highlight the problems and to show business owners why the forms needed to change. The session will include numerous practical examples of 'before' and 'after' situations along with insights gained from an extensive observational testing program.

F503 FATCA Entity Classification: Developing a Guideline for the Canadian Insurance Industry

The U.S. FATCA legislation has had a profound impact around the world. In this session, we'll talk about the issues Manulife Financial encountered while developing an entity classification form. Key challenges included: distilling complex and technical legislation into manageable and accurate plain language; working with an intergovernmental agreement that had not yet been finalized; and, managing lead times for software development and translation into French, Canada's other official language. As we worked with the Canadian Life and Health Insurance Association (CLHIA), Compliance and Taxation experts from other Canadian insurance companies provided feedback and the form we developed became the industry guideline.

The new version enables BFMA members to also create HTML forms form mobile devices like the Apple iPad and Google Nexus tablets. This session will demonstrate these new techniques and the author and presenter will provide copies of his new Adobe Press book on HTML mobile forms.
